Immunization Requirements
All Idaho schools are required to implement Idaho Code section 39-4801, section 39-4802, and IDAPA 16.02.15 “Immunization Requirements for Idaho School Children.” These codes and rules require all students enrolled and enrolling in Idaho schools to have verifiable documentation of immunizations prior to school entrance. This documentation must show date and type of each dose administered. This must be stamped by a physician or health department. Parental recall of dates and types of immunizations received are not acceptable.

Other CDC recommended immunizations:
- Influenza: All children and adults starting at 6 months of age.
- HPV: All girls age 11-12 years old through age 26.
For students who are in process to complete immunizations, a Conditional Admission form must be completed to enroll in school.
Immunization exemptions are allowed, in accordance with Idaho Code 39-4802, and include medical, religious, and ‘other’ exemptions. Parents/guardians may check the ‘Religious/Other’ exemption provided on the immunization requirements form, or may include a signed written statement regarding religious/other exemptions.
